Fillers and Fat Grafting are only effective above the jaw line. The underlying cause of facial aging is atrophy of the bones and fat resulting in a loss of the contours we associate with youth. These issues are successfully treated with volume replacement; either temporary fillers or more permanent fat grafting. Both are excellent fillers but fat slows the aging process and improves skin quality. However, both are effective only above the jaw line.
Botox or Dysport can help maintain and protect a youthful neck contour. If the skin quality (elasticity) is good and the underlying muscle fascia is functional, then Botox® or Dysport® can be used to protect and maintain the shape and relaxation of the platysma muscle, maintaining the youthful neck contour.
Liposuction of the neck should be approached with extreme caution. Some patients complain about having inherited “mom or dad’s neck.” Their face may remain quite youthful, but their neck is genetically full. If the fullness is due to subcutaneous fat alone, conservative lipoplasty can be performed. However, I must raise the caution that improper diagnosis, a limited operative menu or simply the fact that most patients understand and are comfortable with a liposuction option makes it, in my opinion, an overly performed operation. It is vital to maintain the delicate specialized fatty layer just below the skin which maintains a healthy and youthful smoothness as we age. Premature or overly aggressive removal leads to accelerated aging of the neck skin.
Ulthera® provides a non surgical neck lift option. Recently, Ulthera® has been a highly effective non-surgical, option for smoothing and contouring the neck. The shape and support will not be as dramatic as with a surgical neck lift, but there is no downtime associated with the procedure. It also works well with fillers above the jaw line as a non-surgical facial rejuvenation option.

The result is a flat submental plane, from chin to “Adam’s apple”, parallel to the floor.
